Go to Register My Athlete at www.registermyathlete.com and create an account for your student.  Please follow the step-by-step instructions, read and e-sign the documents.  For your convenience, we have attached the physical documents that will need to be printed, filled out, signed by your doctor and uploaded to the website.

2024/2025 AIA Physical Forms (Required to be uploaded to RMA)

Brainbook & Opioid Education must be completed with the certificates uploaded to www.registermyathlete.com.
